school-based immunization update

School-based immunizations, including Meningococcal ACYW-135, Hepatitis B & HPV-9, will NOT be offered in our family practice this Spring 2023. 

Halton Region Public Health will be offering school-based immunization clinics in Spring 2023 to Grade 7 and 8 students.

Reporting Immunizations

Parents/legal guardians must report their child's immunization record to Halton Region Public Health online when they receive any vaccine(s), unless the vaccine is administered at a Halton Region Public Health clinic. Click on the 'Report Immunization' button below to report your child's immunization for Halton Region Public Health.
